Community Mental Health Team provides a wide range of support for people seeking specialist assessment and treatment for a mental health difficulty.

Gateshead Community services cover a wide range of services and provide care for Children and Adults.

The Community Stroke Rehabilitation Team (CSRT) help and support the rehabilitation (recovery and adjustment to life) of people who have had a stroke. We also help and support families and carers.

The discharge liaison team support complex discharges from hospital back to the community.

Eastwood Intermediate Care Service provides a period of rehabilitation to help you learn or re-learn skills which will help you to continue living in your home.

The Falls and syncope service provides specialist assessment, advice and treatment to patients in clinics and in their own home environment to identify and minimise falls risk.

Working in partnership with NEAS, we provide a service to offer timely support to patients at home who have experienced a recent fall.

The service provides post-diagnostic support and treatment for patients aged 65 and over who have been diagnosed with a dementia

The hospice at home service is to provide additional support of the existing community nursing teams. This service operates on a needs-led basis to support people to be cared for and die at home if that is their wish.

The rapid response team provides timely support for patients at home to prevent deterioration in health and well-being promoting independence and avoiding the need for hospital or long-term care.

The Gateshead Strength and Balance Service is aimed at over 65 years old, vulnerable, mildly, and moderately frail adults with a history of falls, fear of falling, reduced confidence and strength and balance deficits.

This service is to provide a supportive service to maximise independence and keep our patients are as well as they can be.
