Falls Rapid Response Service

Working in partnership with NEAS, we provide a service to offer timely support to patients at home who have experienced a recent fall.

About the service

Working in partnership with NEAS, we provide a service to offer timely support to patients at home who have experienced a recent fall.  A combined team of an Occupational Therapist (OT) and a Paramedic complete medical and functional assessments in the patient’s own home referring on to other services and agencies as appropriate, aiming to keep the patient safe at home. 

Staff may arrange for further medical review, or rehabilitation assistive equipment in a bid to minimise the risk of further falls and support people to live as independently as possible.

Our Team

The occupational therapist works alongside the paramedic, providing skilled functional assessment to support people to be as safe and independent as possible at home and working prevent further recurrence of falls.

Referrals

The referral process is made to the team directly through NEAS (North East Ambulance Service).

Other referrals are made via the Rapid Response team via 0191 445 8400.

Service opening times

The service operates 7.00am – 7.00 pm every day and are able to access Bensham Hospital and the Ambulance Stations.
