Eastwood Intermediate Care Service provides a period of rehabilitation to help you learn or re-learn skills which will help you to continue living in your home. This can include improving mobility, personal care, medication or meal preparation.

The Eastwood health team support people recovering from an illness or injury requiring a period of recovery, rehabilitation and or reablement which does not need to be delivered in hospital but cannot be provided in an individual’s own home.

Eastwood also provides assessment beds that are provided by the local authority for people requiring a period of sustained assessment to determine how best their long term needs can be met.

An allocated key worker will discuss interests, hobbies and the activities you may wish to participate in, they will help to build your confidence and will help to co-ordinate your care whilst in Eastwood.

The MDT monitors progress and facilitates therapy and a safe transfer of care to an agreed destination – the goal being the person’s home.

The team

The team consists of Occupational Therapists, Physiotherapist, Nurses and a Rehabilitation Assistant.

The nurses work independently from PGD’s and all have a clinical skills qualification. The team works with the local authority and the GPs.
