Stoma Team Discharge

When you go home from hospital

The stoma team will refer you to your community stoma nurse on discharge. We will also write to your GP.

A member of the QE stoma team will ring you within two working days after discharge to ensure you are managing at home.

Your Community Stoma Nurse will contact you to arrange your first visit within five days of discharge. After your initial assessment, further visits will be discussed with you depending on your needs and contact numbers given.

You will receive an appointment by letter to attend our nurse led stoma clinic  six months after your surgery. Please bring a change of pouch with you. If you are unable to attend, please contact us to rearrange this for you.

A further appointment will be arranged for you to have a telephone appointment with QE hospital stoma nurse twelve months after your surgery.

Following this, you will be discharged from regular follow up by the QE Hospital stoma team.

Annual reviews will be arranged with your community stoma nurse after your first year.

If you have any concerns between appointments (as detailed below), please do not hesitate to contact the Community Stoma Nurse or ourselves during our working hours. Please leave a message on the answer phone if no answer and one of the team will respond to you as soon as possible.

Important note

Should you be admitted into hospital at any time, please remember to bring in your stoma supplies with you or ask a family member to bring them in for you.

This is because we do not have a record of what products you use and may not have your products in our supply cupboard.  Thank you.
